
For your convenience we also offer longer-term payment arrangements through CareCredit Healthcare Finance. Please speak with our patient care coordinator to arrange for payment prior to beginning treatment.
In order to serve you better and keep the cost of audiologic care down, we try to maintain an efficient appointment system. However, our cost of providing care increases greatly when people fail to keep scheduled appointments or cancel at the last minute. We do charge a fee when people fail to keep their appointments or cancel without at least 24 hours notice. This charge is equivalent to our overhead expense for the amount of time scheduled.
If you present a check for insufficient funds, or place a stop payment on an issued check, you will be charged a $30.00 fee for processing.
WELCOME! Our goal is to provide you and your family with optimal audiologic care. We want you to feel welcome and as comfortable as possible during your treatment. We encourage your to ask questions and to get involved with treatment decisions. This includes understanding your treatment plan and as well as our financial policy.
Our office understands the value of insurance benefits to our patients. We are happy to submit the claims necessary to see that you receive the full benefits of your coverage; however we cannot guarantee any estimated coverage. Because the insurance policy is an agreement between you and the insurance company, we ask that all patients be directly responsible for all charges. Please know that we will do everything possible to see that you receive the full benefits of your policy. If for some reason your insurance company does not pay their portion, you are responsible for payment.
We will accept assignment on claims for both primary and secondary insurance. All deductibles, co-pays, and estimated fee amounts not covered by insurance are due at the time of treatment, unless prior arrangements have been made in advance.
In order to honor any insurance benefits, you must provide insurance identification (i.e. insurance cards, completed claim forms, benefit books, etc.) at each visit, and we must be able to verify the current benefits available.
Although we are not a party to the contractual arrangement with your insurance company for hearing aids, we do want to help you receive this maximum benefit. We will gladly provide any procedural codes or written diagnostic reports should your insurance company have any questions about the treatment provided. You can expect the insurance company to reimburse you for a portion of our fee.
If you have two forms of benefit coverage, we will consider your coverage as primary and your spouse's coverage as secondary. If your dependent has two plans, we will apply the Birthday Rule to determine the primary and secondary coverage. If you have two managed care plans or discount plans, your must choose which of the two plans you would like to use.
